Renewal of the Entitlement Form

One of our members asked about the renewal of the Entitlement Form and where to send for the renewal.

You need to send it to St Luke’s Hospital, you can send it up to three months prior to the renewal date and it is now valid for 3 years.

If however you need to renew your Yellow card you have to go in person, again to St Luke’s Schedule office, which is situated through the gates to the right and up the slope, you will need to take with you the new prescription signed by your consultant and your existing yellow card.  I was told this office is open every day (excluding weekends)up until 1pm and it is manned by 6 Ladies who are very efficient, you may also renew your Entitlement form there in person at the same time and it is done whilst you are there.  I always recommend that you take your ID card as well.

    May I ask a Qustion please, I have a British passport however have never lived or worked in the UK as Australian born to English parents.

    I relocated to Malta and have lived here for the past five years.

    To date I have not submitted an entitlement form nor have I applied for a yellow card as I don’t believe it is possible as I have not lived or worked in the UK and have no UK social security number.

    May I ask you to advise if this correct.
